AUTHORIZATION FORM

1- I hereby declare that the above information is true and independent of it being true or not it is my wish to undergo the Microblading through pigmentation or the Depigmentation
2- The procedure and all its’ implications have been clarified and informed to me and I am aware of my psychological and health conditions. I am aware that the Microblading Artist bares no responsibility for any reactions that may occur.
3- I hereby declare that I’m not a patient at risk, I do not have any infectious disease, no immune weakness nor am I dependent of drugs or alcohol.
4- I take all of the responsibility over the post Micropigmentation procedure, as I will follow all of the directions given.
5- Before beginning the procedure the Microblading Artist will show you and inform you all of the products and pigments that have been selected for your procedure.
6- I hereby certify that all of the above have been discussed and that there is no doubt or issue in relation to its’ content and objectives. That being said, I sign off below.
7- I give permission to freely use any pictures or images taken during the procedure, for advertising purposes in any kind of media.
8- The items I don’t agree with are crossed off in ink and are not valid in this document.
